2024 PWC_Panthers_Elite Handbook

Handbook Acknowledgment & Disclaimer *

By signing above, I acknowledge that I have received, read, understood, and agree to comply with the PWC Panthers Handbook. I understand that the Organization has the maximum discretion permitted by law to interpret, administer, change, modify, or delete the rules, regulations, procedures, and benefits contained in the handbook at any time, with or without notice. No statement or representation by any club member, whether oral or written, can supplement or modify this handbook. I agree to comply with, and enforce, ALL the rules, policies, and procedures set forth in the PWC Panthers Handbook.

Policy: Assumption of Risk *

I hereby grant permission for my child to participate in track and field training conducted by the PWC Panthers Elite Track & Field Club. I realize that participation in track and field involves risk of injury; possibly paralysis and even death.

Policy: Release of Liability *

I release PWC Panthers Elite Track & Field Club and its members from any and all liability which might be incurred during the conduct of this activity. I further agree to indemnify and hold the Organization harmless from any claims of lawsuits brought by or on behalf of my child(ren).

Policy: Medical Emergencies *

If I cannot readily be reached during a medical emergency, I hereby grant permission to PWC Panthers Elite Track & Field Club Board members to utilize the most convenient volunteer rescue squad vehicle or ambulance to transport my child(ren) to the hospital at my expense. I authorize medical treatment for my child(ren) at my expense. I hereby verify that my child(ren) has/have passed a medical examination within the last twelve (12) months and can participate in the sport of track and field.

Athlete Photo/Information Release *

Yes. I grant PWC Panthers Elite Track & Field Club permission to photograph and publish my child(ren)'s information, to include name and age, on the organizational website and social media accounts.
No. I do not grant PWC Panthers Elite Track & Field Club permission to photograph and publish my child(ren)'s information.
